Social Services

Our social services team is ready to provide residents and family members with essential information, manage any requests and/or concerns, coordinate any needed appointments, maintain contact and open communication between the facility, families and the residents, and assist with care planning and discharge planning for each resident.

We can also arrange professional services for:

• Dental
• Vision
• Podiatry
• Dementia assessment
• Psychosocial and Psychiatric Evaluation
• Individual therapy
• Arrange transportation to appointments
• Accompanying patients to appointments upon request
